想雷射治療近視、老花，你該知道的事

近視、散光及老花眼，是許多現代人的困擾，

雖然配戴眼鏡可矯正視力，但不少愛美族群想做雷射手術，

免去戴眼鏡的不便及對外觀的影響，

只是有些人接受雷射手術後，卻出現嚴重的乾眼症、眩光，

該怎麼評估自己適不適合透過雷射矯正視力？

美雲原本就有500多度的近視及中度散光，平時就要戴眼鏡才能清楚的看到景物，但隨著年紀逐漸增加，看近物時竟然開始出現不清晰、無法精準對焦的狀況，原來45歲的她已開始步入老花一族。愛漂亮的美雲，不想配戴厚重的老花眼鏡顯老態，雷射能一併解決近視、散光及老花的問題嗎？

飛秒併準分子雷射是

近視手術的主流

近視雷射手術在世界各國已行之有年，技術也都很純熟。臺大醫院眼科部主治醫師王一中表示，近十年來以飛秒雷射合併準分子雷射為近視手術主流治療方式。

飛秒雷射就像一把光刀，可將眼角膜切割並掀開，而準分子雷射則在角膜內層進行氣化削切，之後蓋回角膜瓣，讓傷口自動癒合。丘子宏眼科診所院長暨臺灣大學醫學院眼科副教授丘子宏指出，在飛秒雷射還未問市前，是以板層刀來進行眼角膜的切割，但機械刀不像雷射光刀一樣快又準，角膜瓣切割的厚度較難精準掌握，切得太薄可能讓角膜瓣破損，無法完成近視手術，切得太厚又可能導致剩餘的眼角膜厚度不夠，將來可能因眼壓而讓角膜往前突出，造成「圓錐角膜」病變。不過，上述後遺症在眼科普遍使用飛秒雷射的情況下，目前已很少發生了。

大約3、4年前開始發展出「全飛秒雷射（ReLEX－SMILE）」，跟飛秒併準分子雷射手術的差別是，全飛秒不需將角膜切割開來，也無需使用準分子雷射，手術方式是在眼角膜中間挖除一些組織，傷口癒合後就能達到矯正視力的效果。丘子宏副教授形容被挖除的組織形狀就如同鏡面一般，醫師可根據需矯正的度數，來決定鏡面組織的大小。

全飛秒雷射的好處是不需將角膜瓣切割開來再蓋回去，相對而言，安全性提高一些。不過，飛秒合併準分子雷射在近視手術的準精度、安全性等方面，累積很多醫師多年經驗和大量的病例數，且醫師操作經驗也相當足夠，因此目前還是最多醫院使用的手術方式。

雷射也可矯正老花

在名人代言下，許多中年人對於使用雷射手術來擺脫老花的困擾十分好奇，也躍躍一試。其實矯正老花的雷射並非新技術，醫師也是使用飛秒併準分子雷射來矯正老花，只不過打的模式不同。

■多焦點雷射

丘子宏副教授解釋，老花雷射通常有多焦及雙焦二種模式。多焦削切其實是一種折衷的方式，假如病患有近視加老花，只進行單純的近視雷射手術，術後看遠可達到視力1.2，但看小字很吃力；若使用近視加老花的多焦模式，看遠可能只能達到0.8，但看近就會比之前好很多，不過，選擇這種方式的病患要有心理準備，術後看遠看近雖然大致上沒問題，卻都無法看得十分精準。

■雙焦點雷射

至於雙焦模式，則是將一眼近視完全矯正，另一眼保留75～150度左右的近視度數。

王一中教授補充，完全矯正近視的一眼稱為「優勢眼」，術後主要是用來看遠，而非優勢眼則方便看近。例如病患有500度近視，醫師判斷右眼為「優勢眼」，手術矯正後右眼會有不錯的看遠能力，而被保留些許近視度數的左眼，就成為看近或閱讀的主要視力，雙焦模式對於一般人日常使用都很足夠。

不管是雙焦或多焦削切，皆無法達到雙眼都看得相當精準的程度，單純老花的人想用雷射矯正，就必需犠牲一些看遠的視力，這也是為什麼沒有近視的老花族群很少選擇雷射手術的原因。
